Moving to Episodic Care
We're moving away from traditional primary care services and are creating more flexible, accessible ways to connect — including expanded telehealth visits so you can reach us from wherever you are. Same compassionate care. More ways to receive it.
Our services will focus on evaluation and stabilization of acute illnesses, and care navigation to improve access to care. Patients will be referred to appropriate primary or specialty care providers for ongoing treatment and follow-up care.
