Monday–Friday: 8:00 AM to 5:00 PM.
Closed on weekends and major holidays.
What to Bring to Your First Visit
To ensure a smooth and successful first appointment, please bring:
Valid photo ID (driver's license or government-issued)
Current insurance card (Medicaid, Medicare, private insurance)
A list of all current medications
Any medical records from previous providers (optional but helpful)
Vaccination records (if applicable, especially for children)
School or sports physical forms (if needed)
Arrive 15 minutes early for check-in and paperwork.
